अभ्यर्थियों के लिए निर्देश –

1. आवेदन शुल्क (सभी पदों के लिए) – रु. सामान्य और ओबीसी उम्मीदवारों के लिए 250/- (दो सौ पचास रुपये मात्र) और रु. एससी/एसटी उम्मीदवारों के लिए 50/- (पचास रुपये मात्र) डिमांड ड्राफ्ट के माध्यम से

कोल्लम में देय ईएसआई बचत निधि खाता संख्या 2 के पक्ष में।

2. दिव्यांग/महिला आवेदकों को आवेदन शुल्क से छूट दी गई है।

3. जो लोग एससी/एसटी/ओबीसी वर्ग से हैं, उन्हें तहसीलदार/राजस्व मंडल अधिकारी द्वारा जारी वैध जाति प्रमाण पत्र प्रस्तुत करना चाहिए। ओबीसी प्रमाणपत्र भारत सरकार के निर्देशों के अनुसार निर्धारित प्रपत्र में होना चाहिए जो प्रमाणित करे कि उम्मीदवार क्रीमी लेयर से संबंधित नहीं हैं।

4. चयन योग्यता, अनुभव और साक्षात्कार के आधार पर होगा। साक्षात्कार ईएसआईसी मॉडल और सुपर स्पेशलिटी अस्पताल, आश्रमम, कोल्लम में आयोजित किया जाएगा। आवेदकों को निर्धारित प्रोफार्मा में आवेदन, जाति प्रमाण पत्र (यदि लागू हो) सहित सभी मूल दस्तावेज, सभी मूल दस्तावेजों की ज़ेरॉक्स प्रतियों का एक सेट, दो हालिया पासपोर्ट आकार के फोटो (एक आवेदन पर चिपका हुआ) और अपेक्षित आवेदन शुल्क साथ लाना चाहिए। साक्षात्कार के समय डिमांड ड्राफ्ट का फॉर्म।

5. जिन उम्मीदवारों ने पहले ही किसी केंद्रीय संस्थान में 3 साल की सीनियर रेजीडेंसी पूरी कर ली है, वे 3 साल/1 साल की सीनियर रेजिडेंट पद के लिए पात्र नहीं हैं।

6. अंशकालिक विशेषज्ञों को छोड़कर किसी भी प्रकार की निजी प्रैक्टिस की अनुमति नहीं होगी।

7. “साक्षात्कार” के लिए कोई टीए/डीए स्वीकार्य नहीं होगा।

8. ऊपर बताई गई रिक्तियां साक्षात्कार की तिथि पर वास्तविक आवश्यकता के अनुसार बदल सकती हैं।

9. किसी भी रूप में प्रचार करना अयोग्यता होगी।

10. चयनित विशेषज्ञ/सुपर स्पेशलिस्ट के पास एक व्यावसायिक क्षतिपूर्ति नीति होनी चाहिए

सेवाएँ। प्रति वर्ष न्यूनतम बीमा राशि (1) एनेस्थिसियोलॉजिस्ट, कार्डियक सर्जन, न्यूरो सर्जन, प्लास्टिक सर्जन आदि के लिए 30 लाख रुपये होनी चाहिए। सामान्य सर्जन, स्त्री रोग विशेषज्ञ, प्रसूति रोग विशेषज्ञ आदि के लिए 20 लाख। (3) चिकित्सकों, दंत चिकित्सकों, रेडियोलॉजिस्ट, रोगविज्ञानी आदि के लिए यह कम से कम रु। होना चाहिए। 10 लाख. बीमा और प्रीमियम की एक प्रति और रसीद सगाई की तारीख से 7 दिनों के भीतर जमा की जानी चाहिए।

11. नियुक्ति प्राधिकारी के पास यह अधिकार सुरक्षित है कि वह किसी भी समय, यहां तक कि कार्यकाल से पहले भी, बिना कोई कारण बताए, एक महीने का नोटिस देकर या किसी अनुशासनात्मक कार्रवाई की स्थिति में नियुक्त व्यक्ति की सेवा समाप्त कर सकता है। इसी प्रकार, नियुक्त व्यक्ति को इस्तीफा देने से पहले एक महीने पहले नोटिस देना होगा या नोटिस अवधि के बदले में एक महीने के पारिश्रमिक के बराबर राशि का भुगतान करना होगा।

12. नियुक्ति प्राधिकारी के पास बिना कोई कारण बताए प्रस्ताव/विज्ञापन को रद्द करने का अधिकार सुरक्षित है।

13. चयन के मामले में, उम्मीदवार को 200/- रुपये के गैर-न्यायिक स्टांप पेपर पर चिकित्सा अधीक्षक मॉडल और सुपर स्पेशलिटी अस्पताल आश्रमम के साथ एक समझौता करना होगा। चयनित उम्मीदवार को शामिल होने के समय कोल्लम में देय ईएसआईसी खाता संख्या 2 के पक्ष में डीडी के रूप में 15000/- रुपये की सुरक्षा जमा राशि जमा करनी होगी, जो अनुबंध के सफल समापन और नो-ड्यू सर्टिफिकेट के उत्पादन के बाद वापस कर दी जाएगी।

14. केंद्र सरकार/राज्य सरकार के संस्थानों में काम करने वाले आवेदकों के मामले में, एनओसी होनी चाहिए

संबंधित प्राधिकारी से प्राप्त करें और उचित समय पर जमा करें।

15. एक ही समय में दो ईएसआईसी अस्पतालों में एक संविदा कर्मचारी की अंशकालिक नियुक्ति की अनुमति नहीं होगी।

16. यदि अधिसूचित PWD सीटों के कैडर में कोई उपयुक्त उम्मीदवार उपलब्ध नहीं है, तो DOPT कार्यालय ज्ञापन संख्या 36035/02/2017-Estt (Res) दिनांक 15 जनवरी, 2018 के अनुसार निर्धारित निर्देशों का पालन किया जाएगा।

Instructions for candidates –

1. Application Fee (for all posts) – Rs. 250/- (Rupees Two Hundred Fifty only) for General and OBC candidates and Rs. For SC/ST candidates, Rs 50/- (Rupees fifty only) through demand draft.

2. Disabled/female applicants are exempted from the application fee.

3. Those who belong to SC/ST/OBC category should produce valid caste certificate issued by Tehsildar/Revenue Divisional Officer. The OBC certificate should be in the prescribed form as per the instructions of the Government of India certifying that the candidate does not belong to the creamy layer.

4. Selection will be based on qualification, experience and interview. The interview will be conducted at ESIC Model and Super Specialty Hospital, Ashramam, Kollam. Applicants should bring application in the prescribed proforma, all original documents including caste certificate (if applicable), one set of Xerox copies of all original documents, two recent passport size photographs (one affixed on the application) and requisite application fee. . Demand draft form at the time of interview.

5. Candidates who have already completed 3 years Senior Residency in a Central Institute are not eligible for 3 years/1 year Senior Resident post.

6. No private practice of any kind will be allowed except for part-time specialists.

7. No TA/DA will be admissible for “Interview”.

8. The vacancies mentioned above are subject to change as per the actual requirement on the date of interview.

9. Canvassing in any form will be a disqualification.

13. In case of selection, the candidate will have to execute an agreement with the Medical Superintendent Model and Super Specialty Hospital Ashramam on non-judicial stamp paper of Rs.200/-. The selected candidate will have to deposit a security deposit of Rs.15000/- in the form of DD in favor of ESIC Account No. 2 payable at Kollam at the time of joining, which will be returned after successful completion of the contract and production of No-Due Certificate.
